Scientists of the Department of Oncology of IFNMU at the Precarpathian Clinical Oncology Center for the first time performed a complex combined operation in oncopathology of the maxillofacial area with its plastic restoration using a bone graft.
The operation was performed by Associate Professor Ivan Kostyshyn , Professor Volodymyr Holotyu and assistant Halyna Hirna. Anesthesiological support of the operation was performed by anesthesiologist Ivan Kasyuk and anesthesiologist Vasyl Zagursky. The operating room nurse is Olga Vintonyak , informs “Vikna” with reference to the press service of Ivano-Frankivsk National Medical University.
According to scientists, in a patient with a tumor of the bottom of the oral cavity, which grew into the lower jaw and metastases to the lymph nodes of the neck, the removal of the tumor process was possible only as a result of consistent application of a number of high-tech treatments. At the first stage, the patient received preoperative chemoradiation therapy. The next step was an eight-hour surgery. It consisted of the removal of metastatic lymph nodes on both sides of the neck and the actual tumor of the bottom of the mouth. In this case, in order to ensure the radicality of surgery, a thorough resection of two thirds of the lower jaw was performed.
A large defect of the maxillofacial area, which was formed as a result of surgery, in the future could lead to a violation of many vital functions: breathing, eating, speech, unsatisfactory mental state. That is why the reconstructive stage of surgical treatment was performed at the same time. Today, many methods are known for plastic closure of mandibular defects, but their use in cancer patients is limited due to the negative impact of radiotherapy on graft engraftment. Given all the advantages in this clinical situation, the defect of the mandible was successfully removed with a bone graft from the iliac wing, followed by plastic formation of the bottom of the mouth.
Adherence to the principles of radical surgery, followed by bone grafting, has improved the patient’s oncological prognosis and restored functional and cosmetic disorders, according to the Department of Oncology. In the future, this will allow a faster return to active social life, doctors of IFNMU are convinced.
